Depending on the programme you've chosen, you may need to fill out a few extra forms upon submitting your application. Please find reference to these below.

Ākonga under 16 years of age

Please complete the Ākonga under 16 years of age: Support of a parent/guardian/caregiver form. This form can be downloaded below or you can get copies of this form from Ask Me! student services centre. Once completed, please submit it here or bring the original into an Ask Me! student services centre.

Late enrolment declaration form

Please complete the Late enrolment declaration form.  This form can be downloaded below or you can get copies of this form from Ask Me! student services centre. Once completed, please submit it here or bring the original into an Ask Me! student services centre.

Privacy Authorisation form

Please complete the Privacy Authorisation Ākonga (Student) Information form if you would like to give someone access to your information or make changes on your behalf. This form can be downloaded below or you can get copies of this form from Ask Me! student services centre. Once completed, please submit it here or bring the original into an Ask Me! student services centre.

Building and civil construction

Please complete an employer form for the New Zealand Certificate in Carpentry (Level 4) programme. For the New Zealand Certificate in Construction Trades Supervision (Level 5) programmes you'll need to complete both the employer and specialisation forms.

These forms can be downloaded below or you can get copies from Ask Me! student services centre. Once completed, please mail to the address provided on the form, bring the original into an Ask Me! student services centre, or use the online file upload service.

Education

Please complete two referee forms. This form can be downloaded below or you can get copies of this form from Ask Me! student services centre. Once completed, please mail to the address provided on the referee form or bring the original into an Ask Me! student services centre.

At least one referee must be from a professional body and/or organisation.

    If you have recently lived in Australia:

    Maritime

    Forms can be downloaded below, or you can get copies of this form from Ask Me! student services centre. Once completed, please submit it here or bring the original into an Ask Me! student services centre.

    For entry into the New Zealand Diploma in Marine Engineering (Level 6) or the New Zealand Diploma in Nautical Science (Level 6), applicants need to complete this Maritime New Zealand medical declaration form.

    Once completed, please submit it here or bring the original into an Ask Me! student services centre.

    For entry into the Diploma in Foreign-going Master or Chief (Level 7) Chief Mate/Master or Chief Engineering Foreign-Going, applicants need to complete this Maritime New Zealand medical declaration form.

    Once completed, please submit it here or bring the original into an Ask Me! student services centre.

    As part of the New Zealand Diploma in Nautical Science, ākonga must study STCW Proficiency in Survival Craft (PISC). The pre-requisite for this paper is proof of 12 months of sea service. This is the only paper in the programme where completion of the Certificate of Sea Service is required.

    Nursing
